The best you can do is pay really close attention to all of the landmarks and try to line them up on your map.
I've only had to do this a couple of times, but sometimes you just have to use a serpentine search pattern: pick a spot and go west for while, then go north for a short time, then east until you hit the coast, then north, west, north, east, etc until you find it. If your map doesn't have a red X on it, your game is bugged and you may not be able to find the hideout. All Pirates!

To be able to find Montalban at sea at all, you'll need to get a lead on him just like with Raymondo, except the lead is a little harder to get. You must: - Rescue a missing family member which in turn requires you going through the whole Raymondo chase several times ; - Dance near-perfectly with a beautiful governor's daughter. Either will get you Montalban's location. You will then need to chase his Flag Galleon in exactly the same way you track down Raymondo; talk to bartenders and crews of captured ships to find out where he's gone off to next.
